Assessing and Addressing Social Determinants of Health

Course overview

Growing evidence on the impact of social determinants of health (SDOH) calls for change both within and outside the healthcare system in order to improve health outcomes and eliminate health disparities. Clinicians often lack the tools and resources needed to screen for and address SDOH within their patient populations. In this course, clinicians will gain applicable and relevant tools and resources for screening for and mitigating the impact of social determinants of health. Change in both clinician practice and healthcare systems to address SDOH has potential to decrease health disparities in the US. This course will also discuss advocacy for change outside the healthcare system.
