Course overview
Coordination of care and services is a fundamental tenant of safety and quality in home care. The process may be compared to the juggler that must keep the plates in the air spinning by making sure all parts are performing correctly. Example patient case scenarios will be presented that show effective care coordination, communication, and documentation. Communication with physicians from a regulatory perspective will be emphasized. This course provides examples, understanding, and solutions to achieve coordination of care and services while delivering quality care to the patient.
