Course overview

This course aims to demystify the discharge process through current evidence-based factors which will help guide successful discharges and outcomes for our patients. Therapists have a unique role in facilitating function and training to promote safety at discharge with repeated data focusing on medication management ADLs and mobility. Testing tools and developing plans based on unique patient diagnoses will be applied to improve clinician skills for functional carryover and best-practice approaches to discharge planning.
