Social determinants of health have taken center stage in recent health policy discussions. There is a growing trend to improve population health initiatives. Health care providers are increasingly being asked to measure impact in terms of the health outcomes of the population they serve. Social determinants of health operate at multiple levels. Underlying structural factors, such as policy, transportation, and broader cultural and institutional contexts shape the distribution of resources across society and people's social position within it. This course will include a discussion on social determinants of health and theories behind it, and will discuss why they are important to improving population health. It will also lead participants to be able to recognize some areas of improvement for the communities they serve, and see examples of how it has been applied on a daily basis in other communities. Suggested audience: All health care providers and other community stakeholders Upon completion of this presentation, you should be able to: Define social determinants of health Describe the theories behind social determinants of health Identify why social determinants of health are important to improving population health Recognize some areas of improvement for the communities your hospital serves Learning Outcomes Standard: Based upon definitions by the Centers for Disease Control (CDC) and World Health Organization (WHO) and various theories supporting social determinants of health.
