Leveraging Technology to Reduce Readmissions for Super Utilizers

Course overview

This session provides insight into how one health system leveraged the electronic medical record to create multidisciplinary readmission prevention plans that were triggered upon the patient's presentation to the emergency department. These plans helped the emergency department personnel take appropriate actions to prevent unnecessary admissions. This intervention was successful in driving down readmissions by more than 50% for the organization's top 150 super utilizers.
