TRANSITIONAL CARE PLANNING FOR CANCER PATIENTS (2 CE)

Course overview

Optimal care for cancer patients is dependent on careful planning across different care settings to ensure a continuum of care. Patients now move across numerous care settings during treatment (e.g., the transition from being an inpatient at a hospital to being an outpatient in need of home care). Key components to successful outpatient and home care treatment of patients with cancer are a coordinated team for delivery of health care, an involved and committed patient and/or family, and the availability of ongoing support and education for the patient and family. Learning Objectives: 1. Review the key components for transitional care for cancer patients. 2. Review the elements of a comprehensive biopsychosocial assessment (physiological, demographics, psychological, social, spiritual, and legal). 3. Review biopsychosocial interventions and options. 4. Review special considerations of transitional care planning, including home care, insurance and financial, in the workplace, and legal considerations. 5. Review issues related to end-of-life decisions.
