Supporting Integrated Care through Case and Care Management

Course overview

People with mental health and substance use disorders are at higher risk for negative health outcomes and health disparities compared to other parts of the population. Many factors can influence an individuals health outcomes, such as comorbidities, the fragmented healthcare system, or social determinants of health. However, there are also strategies to address those factors, mitigate risks, and prevent negative health outcomes. Case management and care management are two approaches that can support integrated care and help clients with multiple needs navigate various systems, reduce healthcare costs, and ultimately improve client health outcomes.
