Course overview

In this care management course, participants learn about using a consistent approach to help improve efficiency while also fine-tuning the process used on a regular basis. Participants learn of the method called "Learn-Create-Manage", which helps simplify a complex process and provide direction, so care managers can feel confident and offer tremendous value to those they serve. Participants learn about the comprehensive assessment and the key areas of focus. Participants learn how to create a plan of care, focusing on the areas of concern, barriers, goal setting, and making recommendations. Presenters review the cost of care and how to assist individuals and families by having a greater level of understanding and preparedness.
