Introduction to patient safety culture, patient injury investigation, and laboratory error and injury classification. Learning Objectives 1. State 2 reasons why most labs do not have a 'patient safety culture'. 2. List 3 major findings from studies of patient safety. 3. List 3 sources of cases of lab-related patient injuries. 4. Describe the barriers to determining patient injuries and techniques for overcoming these barriers. 5. Define cognitive and noncognitive errors. 6. Characterize any case of lab-related injury (or potential injury) regarding: - Patient outcome - Responsibility for problem - Phase of testing - Preventability and error type - Interventions to prevent similar cases
