Patient volumes in the acute care setting continue to rise, with hospitals being at and over capacity on an alarming basis. Therapists in the acute care setting are pushed to complete evaluations and make discharge recommendations faster than ever before; sometimes basing that decision on 15 minutes of face-to-face time. To compound this problem, there is a subset of patients that are the revolving door of misunderstood medical follow-up needs, housing instability, and unmet community resources. This workshop will look to discuss creative ways to approach discharge planning, utilizing other members of the interdisciplinary care team and community resources to aid in addressing the non-traditional patient. This course will discuss patient engagement in discharge planning and establishing realistic goals for the acute care setting.
