0202-0000-24-316-L04-P -- Pharmacy led patient visit to review the patient's overall wellness including emotional and psychological well-being. A personalized preventative health plan is developed to allow the opportunity to help prevent disease and disability based on the patient's current health and risk factors. This is an interdisciplinary program effort involving pharmacy, family medicine, radiology, surgery, audiology, optometry, and others.
