Course overview

Patient-centered care, also known as value-based care, is about transforming the relationship between healthcare providers and patients from the traditional model (same treatment for similar problems) to individualized care. The individualized care provided is based on each patient's unique preferences, background, lifestyle, and medical concerns. This course will focus on recognizing the importance of the patient experience as they transition throughout healthcare care continuum and the role of the patient and staff in designing the organization's environment and systems of care for a positive patient experience. Suggested Audience: Hospital administration, governance board members and current and future leadership staff Upon completion of this course, you should be able to: Identify techniques to focus on the healthcare system's customers. Define the concept of co-design and its importance in healthcare delivery. Identify the components of "Empathy Mapping." Learning Outcome Standard: The content in this course is based upon the framework designed by the Institute of Healthcare Improvement and additional data and statistics, industry standards, and best practice standards derived from multiple nationally recognized sources in Leadership and Patient Engagement.
