OBJECTIVES 1. Describe the following aspects of Heart Failure: Classifications -- Signs and Symptoms Causes and Conditions that may lead to Heart Failure Medical Tests and Findings Treatments 2. Discuss Stanford Universities new CAD treatment system in terms of: 1. The current inflexible, inefficient and costly mode of care delivery 2. The paucity of health information tools (HIT) in CAD treatment practice 3. The current disconnect between the processes of care delivery and outcomes analysis 3. Explain the impact of multi-disciplinary teams and in-person communication on the treatment outcomes of patients with Heart Failure. 4. Assess the effectiveness of non-pharmacological interventions for post-discharge care in heart failure patients. 5. Explain Accountable Care Organizations (ACOs) in terms of their historical context and the Affordable Care Act
