After completing this activity, learners will be able to: 1. Identify the critical steps in medication reconciliation to ensure accuracy in patient care transitions. 2. Understand the risk factors and populations most vulnerable to medication errors. 3. Recognize the common causes of medication errors at different stages of the medication process and strategies to mitigate them. 4. Learn the role of technology in preventing medication errors, including electronic prescribing systems and bar-coding. 5. Develop best practices for error prevention, including clear communication, standardized processes, and patient engagement.
