MAJOR AORTOPULMONARY COLLATERAL ARTERIES (MAPCAS) ARE SYSTEMIC-TO-PULMONARY ARTERIES OF VARIABLE ORIGIN OR SIZE THAT CARRY BLOOD TO THE LUNGS IN CASES OF UNDERDEVELOPMENT OR ABSENCE OF THE NORMAL PULMONARY ARTERIES. THESE ANOMALOUS VESSELS ARE USUALLY ASSOCIATED WITH CONGENITAL HEART DEFECTS, PARTICULARLY THOSE OF VERY COMPLEX TYPES, SUCH AS TETRALOGY OF FALLOT WITH PULMONARY ATRESIA. THE MAPCAS PRESENT CONSIDERABLE VARIATION IN SIZE, NUMBER, AND ORIGIN, SIGNIFICANTLY AFFECTING DIAGNOSIS AND TREATMENT PLANNING. MANAGING MAPCAS IS COMPLEX, OFTEN REQUIRING COMBINING SURGICAL WITH OTHER TREATMENT MODALITIES. THE THERAPEUTIC GOAL IS TO MAKE LUNG PERFUSION CLOSER TO NORMAL THROUGH RECONSTRUCTION OR AUGMENTATION OF THE NATIVE PULMONARY ARTERIES AND, SOMETIMES, OBLITERATION, DISCONNECTION, OR LIGATION OF THE COLLATERAL VESSELS. STAGED REPAIR PROCEDURES INVOLVE INITIAL SURGERIES IN INFANCY, FOLLOWED BY DEFINITIVE REPAIRS IN LATER YEARS. THE TREATMENT APPROACH DEPENDS ON AN INDIVIDUAL PATIENT'S ANATOMY AND GENERAL HEALTH STATUS. REGULAR FOLLOW-UP IS ESSENTIAL TO MONITOR LUNG DEVELOPMENT AND CARDIOVASCULAR HEALTH.THIS ACTIVITY FOR HEALTHCARE PROFESSIONALS IS DESIGNED TO ENHANCE LEARNERS' PROFICIENCY IN EVALUATING AND MANAGING MAPCAS.
