Course overview
Heart failure and chronic obstructive pulmonary disease affect approximately 10 million patients worldwide and are a leading global clinical, social, and economic burden. This session will provide approaches and strategies to assist in the management of this patient population, as well as symptom management, patient education, and population health initiatives. Attendees will obtain education and best practice examples that will assist in reducing unnecessary costs and treatments, while improving patient outcomes and overall quality of life. Suggested Audience: Nursing leaders and managers, care coordination and case management staff, population health staff, quality leaders and managers, and primary care clinical staff. Upon completion of this presentation, you should be able to: Identify and address top symptoms, exacerbation causes, and treatment compliance issues that can appear in this patient population. Recognize care coordination, patient education, and preventative management approaches to implement that will improve patient outcomes and quality of life. Design population health strategies to coordinate acute and primary care, and community efforts to effectively manage this patient population. Learning Outcomes Standard: Based on CMS, NIH, AAFP, ADA, AHA, SAMHSA, Care Coordination, Population Health, and best practice initiatives through State/Federal regulations, standards, and guidelines as they pertain to the management and treatment of chronic diseases. In order to receive 0.1 CEU credit / 1 credit hour for this course, you must: Complete course in its entirety. Complete course within 30 days. Pass all assessments with an 80% or better.
