Tibial stress fractures are a common yet challenging condition in runners, often resulting from rapid load progression and insufficient energy availability. This case study examines the continuum of bone stress injuries, distinguishing between tibial bone stress injuries and medial tibial stress syndrome (MTSS) based on symptoms, risk factors, and management strategies. Clinicians will explore key diagnostic techniques, including percussion tests, palpation, progressive calf loading, and single-leg hopping, alongside the role of imaging modalities in confirming bone stress injuries. The case study also highlights the physiotherapist's role in identifying relative energy deficiency in sport (RED-S) and its implications for recovery. Additionally, rehabilitation strategies focusing on loading progression and safe return-to-running protocols will be addressed to optimize patient outcomes.
