An immune-mediated hemolytic transfusion reaction (HTR) is one of the most feared adverse consequences of a transfusion. Prompt recognition of an HTR can be life-saving. It is important to consider non-immune causes of red cell hemolysis that may mimic an HTR and know how to recognize and prevent these impostors. Knowledge of the basic fundamentals of blood bank testing and red cell compatibility is assumed.
