0202-0000-25-143-L04-P - 0202-0000-25-143-L04-T - Symba Center, a non-profit health and wellness organization located in California, has developed sustainable programs to address substance use disorder, primary care, mental health and housing needs in this underserved population by adopting an interdisciplinary approach through a collaborative care model. Attend this session to learn from the pharmacist who saw a need and worked tirelessly to implement an integrative and multidisciplinary health and social service model for the homeless residents of a Southern California community. He'll share stories and lessons learned along his journey.
