Course overview

Today, a small number of people use a large share of healthcare services. These individuals often have more than one long-term health condition and face repeated medical or mental health crises. Many also deal with challenges like unstable housing, low income, or trouble getting to appointments. These are called social determinants of health, and they can make it harder to stay well. Case managers and care coordinators play a key role in helping this population. They connect people to services, build care plans, and help prevent crises before they happen. This course is designed to support healthcare workers, such as nurses and social workers, who provide care or case management services in payer organizations. This course will provide practical tools and strategies for crisis prevention and care planning.
