As pharmacists, we need to understand all of the potential pitfalls that endanger patient safety. To provide an approach to best protect the patients that we serve, multiple nonprofit, for-profit and government agencies have collaborated to identify potential problem areas, detect medication errors, report them, develop technologies to help avoid missteps and prevent their recurrences, and prevent new errors from occurring.
