Course overview

This lesson is designed for Registered nurses in the long term care setting. At the completion of this lesson learners will be able to discuss the Patient Protection and Affordable Care Act as amended by the Health Care and Education Reconciliation Act, discuss hospital readmission rates currently experienced that involve skilled nursing facilities, describe the joint initiative of the Center for Medicare and Medicaid Innovation (Innovation Center) and the Medicare-Medicaid Coordination Office (MMCO), and list some potential interventions that were suggested by the initiative.
