Millions of Americans face multiple chronic conditions requiring complex skills and treatments. And its not getting better. Because of the increases in acuity and the aging population, all nurses need to possess basic competencies to meet the demands of an aging and diverse society, with an emphasis on clinical training in multiple settings across the lifespan. Transitional Care Nursing is a nurse-led, multidisciplinary care model in the implementation of tested protocols with a unique focus on increasing patients' and caregivers' ability to manage their care. This webinar will discuss this up and coming area of practice and how nurses lead with collaborative advocacy to ensure positive patient outcomes.
