This package highlights the evolving skills consultant pharmacists need to thrive in todays rapidly shifting long-term care landscape, from integrating digital therapeutics and pharmacogenetics to supporting safe transitions of care. Learners will explore timely policy and payment updates, including major Medicare Part D changes, alongside sessions that strengthen leadership, collaboration, and antimicrobial stewardship. The program also delves into expanding pharmacist roles in home-based, age-friendly care and examines critical ethical and equity issues affecting older adults, including disparities in end-of-life options. Together, these activities offer practical, forward-looking insights designed to help pharmacists elevate patient safety, precision medicine, and quality of care across the aging continuum.
