This month, we are reviewing best practices for pharmacists practicing on both sides of the transition from hospital to home. In episode 1, APhA Member Marissa Cavaretta will discuss evidence-based strategies and real-world experiences that elevate a hospital¿pharmacists¿impact at discharge, including medication reconciliation, discharge counseling, and post-discharge follow-up calls aimed at preventing avoidable readmissions.
