THIS INTERPROFESSIONAL COURSE EXPLORES THE RAPIDLY CHANGING LANDSCAPE OF CHRONIC HEART FAILURE MANAGEMENT, WITH A FOCUS ON NEW PHARMACOLOGICAL TREATMENTS, EMERGING DEVICE TECHNOLOGIES, AND THE EXPANDING ROLE OF TELEMEDICINE IN OPTIMIZING PATIENT OUTCOMES. DESIGNED FOR A DIVERSE CLINICAL AUDIENCE -INCLUDING PHYSICIANS, NURSES, AND PHARMACISTS - THIS COURSE EMPHASIZES COLLABORATIVE CARE MODELS, CLINICAL APPLICATION OF GUIDELINE-DIRECTED THERAPIES, AND THE INTEGRATION OF DIGITAL HEALTH TOOLS TO SUPPORT PATIENT-CENTERED CARE.
