Building Better Primary Care for Diabetes

0202-0000-26-047-L04-P Nearly 90% of patients with diabetes utilize a primary care setting for their care and one in three patients seen in a primary care facility has diabetes. Despite these staggering statistics, a person with diabetes (PWD) often faces inconsistent, confusing, and variable care.  American Pharmacist Association (APhA) has partnered with the American Diabetes Association (ADA) and other primary care professional associations to build consensus on what patients should expect from a primary care team and how care providers, including pharmacists can translate the ADA Standards of Care into meaningful practice.  This session will review those standards, identifying key services that have shown to improve diabetes outcomes and introduce a checklist that walks through what people with diabetes should expect from their primary care experience. Through patient case studies and interactive tools, participants will identify ways to optimize their clinical processes and improve diabetes care for patients, including those in low-resource primary care settings.
