0202-0000-26-123-L04-P This session highlights how pharmacists can participate in care models to improve patient outcomes with two real-world examples. (1) The California Right Meds Collaborative (CRMC), launched by USC School of Pharmacy in 2019, brings together health plans, pharmacies, and academic partners to advance Comprehensive Medication Management (CMM) through a sustainable, value-based payment model. This demonstrates how community pharmacies are uniquely positioned to demonstrate value through patient-centered services. (2) A virtual-based pharmacist clinic at Sutter Health helps patients, physicians, and care teams decrease cardiac admissions for heart failure by proactively identifying and treating patients with gaps in care. The speakers will discuss strategies to help pharmacists optimize their ability to provide clinical care and demonstrate their value through patient-centered services.
