What you'll learn
Upon completion of this course, participants will be able to:
1. Describe the principles and importance of care coordination in long-term care settings.
2. Conduct comprehensive resident assessments, including physical, behavioral, and psychosocial domains.
3. Apply evidence-based diagnostic and treatment protocols for common LTC conditions.
4. Integrate pharmacologic management with patient-centered care and monitor for contraindications and adverse effects.
5. Facilitate interdisciplinary collaboration among nursing, PT/PTA, OT, behavioral health, nutrition, social work, and physicians.
6. Develop individualized care plans and educate residents and families to improve adherence and outcomes.
7. Document care and interventions accurately to meet regulatory standards and quality measures.
Course overview
This course provides healthcare professionals with a comprehensive understanding of care coordination in long-term care (LTC) settings. Participants will explore assessment, care planning, interdisciplinary collaboration, pharmacologic management, diagnostic interpretation, and implementation of evidence-based treatment protocols. Emphasis is placed on patient-centered care, quality outcomes, regulatory compliance, and the integration of all allied health roles to enhance continuity of care for residents with complex medical and psychosocial needs.
