The home health care industry is being transformed. Outcome Assessment Information Set Version E (OASIS-E), Home Health Value-Based Purchasing (HHVBP), Patient-Driven Groupings Model (PDGM) and Home Health Care Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) all spell significant change. Effective home health care agencies must integrate a multidisciplinary approach aimed at achieving measurable, meaningful characteristics and abilities useful to the patient, and all within an efficient and effective delivery system. High quality care will be rewarded, and subpar clinical outcomes will have significant consequences. This course is designed to be the health care professional's pathway to success in navigating the "alphabet soup" of the home health care environment. The participant will develop the skills necessary to formulate patient centered care plans linking evidence, intervention, and outcomes. The tools to achieve OASIS precision, value-based outcomes and the optimal patient experience will be provided. Case study scenarios will challenge the participant to synthesize multiple clinical factors in making best practice decisions. Upon completion of this workshop, the participant will have a greater understanding of, and proficiency in, achieving value-based care within the post-acute space.
