At the conclusion of this program, participants will: 1. Describe the role of the Institute for Healthcare Improvement (IHI); 2. Identify the three components of the IHI Triple Aim; 3. Describe at least two gaps in healthcare relative to the older adult; 4. Describe at least two types of morbidity affecting older adults due to healthcare gaps; 5. Define the "4Ms"; 6. Describe at least two ways the 4Ms may improve quality of life and health for older adults; 7. Describe at least two strategies for incorporating the 4Ms in clinical practice; 8. Describe a way in which the 4Ms could be communicated across the healthcare spectrum; 9. Explain how Accountable Care Organizations reflect the IHI Triple Aim; 10. Describe the impact of differing requirements for training on Alzheimer's disease and related disorders among Florida's hospitals, assisted living and skilled facilities, home health and hospice providers.
