Rehabilitation therapists are pressured to provide thorough and precise documentation to prove medical necessity. Additionally, rehabilitation therapists are required to support their billing and coding decisions in their documentation. Changes to documentation systems and to the coding/billing laws occur frequently. Outpatient rehabilitation therapists have an added stressor of a quick and quantity-based environment. After providing patient care, therapists frequently spend several hours at the end of their day documenting, billing and navigating any errors made from prior days' notes.
