This 2-hour CE course reviews incident reporting systems and continuous quality improvement in pharmacy practice. Topics include medication errors, near misses, unsafe conditions, nonpunitive reporting, root cause analysis, contributing factors, team communication, pharmacist and technician roles, and CQI tools such as Plan-Do-Study-Act cycles. Participants will learn how incident reports can identify safety trends, support system-wide learning, and guide practical workflow improvements that reduce recurring errors and improve patient safety.
