Course overview
This interactive course provides Sexual Assault Nurse Examiners (SANEs) with an opportunity to critically review completed medical forensic examinations. This course utilizes actual de-identified sexual assault medical forensic examination records obtained with patient authorization for educational use. All protected health information (PHI) and identifying information have been removed before presentation in accordance with privacy standards and organizational policies. Participants will review real-world documentation examples to strengthen chart auditing skills, identify documentation deficiencies, and improve forensic nursing practice. Participants will evaluate documentation practices, forensic history collection, injury identification, medical treatment, evidence collection, referrals, discharge planning, and overall legal defensibility. Through structured peer review and case discussion, participants will identify strengths, opportunities for improvement, and best practices that enhance patient care and support successful criminal justice outcomes. Accurate documentation is essential to patient safety, continuity of care, reimbursement, quality improvement, and legal protection. Healthcare documentation is frequently scrutinized during regulatory reviews, quality assurance investigations, and legal proceedings. Poor documentation may compromise patient outcomes and increase professional liability. This course examines healthcare documentation through quality improvement and forensic nursing lens. Participants will learn to identify documentation deficiencies, conduct structured chart audits, and apply evidence-based documentation practices that support patient safety, regulatory compliance, and legal defensibility.
