Nociplastic dominant chronic low back pain can be challenging to assess and manage, particularly in injured workers where pain sensitivity, fear avoidance, low self-efficacy, and work-related factors may influence recovery. This case study explores how clinicians can identify nociceptive, neuropathic, and nociplastic pain mechanisms, develop a clear pain formulation, and apply psychologically informed strategies including education, graded activity, goal-based rehabilitation, and multidisciplinary care to support function and return to meaningful activity.
