There is a growing population of patients who have complex illness, frailties, disabilities and resultant functional limitations. Extremely varied in their functionality, this population poses unique challenges to the rehabilitation professional. Within our society, the deleterious and growing effects of immobility, obesity, and chronic disease has complicated the care of the medically fragile patient. Increasing rigor imposed by the health care industry will challenge the rehabilitation professional to better formulate, articulate and successfully execute effective and efficient treatment plans tailored to the individual patient. This one-day workshop will better equip the health care professional to formulate patient centered care plans linking evidence, intervention and outcomes that travel across practice settings. Case study scenarios will challenge the participant to apply prescriptive analytics in making clinical decisions. This evidence-based lecture and hands on lab will expose the clinician to standardized assessment tools and big picture strategies to effectively manage the needs of the medically fragile patient, regardless of their practice setting.
