Course overview

In both skilled nursing facilities (SNFs) and home health (HH), older adults rely on physical and occupational therapy to regain function. In 2024, CMS elevated the importance of functional outcomes by adopting the Discharge Function Score across the HH Expanded VBP Model, SNF QRP, and SNF VBP directly typing payment to patient progress. This session gives PTs and OTs a clear, practical overview of the measure, how it is calculated, and why it matters. Attendees will learn actionable strategies for integrating the score into daily practice, strengthening documentation, and ensuring that each patient's functional gains are accurately captured.
