Insomnia prescribing requires more than choosing a sleep aid, it requires matching drug mechanism and pharmacokinetics to the patient's symptom pattern, comorbidities, and safety risks. This APRN-focused pharmacology course reviews FDA-approved and commonly used off-label options for insomnia, with practical guidance on selecting agents for sleep onset versus sleep maintenance complaints, choosing doses in older adults, minimizing next-day impairment, and reducing harm in patients at risk for falls, respiratory compromise, complex polypharmacy, or substance use. Learners will apply guideline-supported decision pathways, interpret key safety signals and labeling considerations for newer dual orexin receptor antagonists, and use structured tapering and monitoring approaches to reduce long-term hypnotic exposure when appropriate. Pediatric and adolescent prescribing considerations are also addressed, with emphasis on rational selection, monitoring, and risk mitigation. Learning Objectives By the end of this course, the participant will be able to: Differentiate major insomnia medication classes by mechanism of action, onset and duration, and expected clinical effects on sleep onset and sleep maintenance. Select an insomnia pharmacotherapy regimen aligned with patient-specific factors including age, fall risk, cognitive risk, comorbid conditions, and concurrent CNS depressants. Apply evidence-informed approaches to initiating, titrating, and monitoring dual orexin receptor antagonists and other FDA-labeled insomnia agents using prescribing information and guideline recommendations. Identify and manage common and high-risk adverse effects of insomnia medications including next-day impairment, complex sleep behaviors, anticholinergic burden, respiratory depression risk, and drug-drug interactions. Implement deprescribing and tapering strategies for chronic hypnotic use, including patient counseling points and follow-up parameters that reduce rebound insomnia and withdrawal risk.
