Though no healthcare worker intentionally makes a mistake, the truth of the matter is that more preventative thinking and less after-the-fact blaming is the push for safer healthcare. In one of the wealthiest nations in the world, with the most expensive healthcare system in the world...the U.S. continues to rank embarrassingly low on health and morbidity. It is estimated that Medical errors account for over 200,000 deaths annually, and those numbers could actually low as they focus on the hospital setting and don't account for error deaths in Nursing Homes, ALF's, Rehab centers and patient homes. What can be done?
