0202-9999-26-254-H01-P This interactive BCGP Recertification case study will provide pharmacists with the opportunity to apply knowledge related to pharmacist interventions during transitions of care. Optimal care management, and access and adherence barriers will be discussed as this case follows a patient from admission to discharge, with consideration of pharmacist interventions to address access and adherence barriers while supporting continuity of care.
