Clinical Documentation SOAP Notes and Recordkeeping for Massage Therapists is a two instruction hour online continuing education course for Florida massage therapists. The course explains how accurate, timely, objective, and confidential documentation supports ethical practice, client safety, communication, continuity of care, and the prevention of medical errors. Students will study client intake assessment, informed consent, SOAP notes, treatment planning, progress evaluation, confidentiality, record security, record release, retention requirements, error correction, late entries, and risk management. The course emphasizes the difference between massage assessment and medical diagnosis. It also explains how incomplete, inaccurate, or unclear documentation can contribute to treatment errors, communication failures, privacy violations, and other risks. The course is presented as a self-paced online lecture divided into six chapters. Students must read all course material and complete a twenty-question multiple-choice final examination. A minimum score of eighty percent is required for successful completion.
