Documentation Boot Camp: Forging Documentation Warriors is a 3-hour continuing education course for physical therapists, occupational therapists, and speech-language pathologists focused on strengthening the clinical, regulatory, and ethical foundations of documentation practice. Participants will learn to establish medical necessity, write SMART goals grounded in evidence-based practice, apply current Medicare and payer documentation requirements, and approach the SOAP note as an ethical act that captures both objective clinical findings and the patient's own voice.
