Course overview

This learning activity prepares Certified Nursing Assistants to function as effective Assessment Allies by developing the observation, reporting, and documentation skills that support clinical excellence in daily patient care. Through practical guidance and real-world examples, the activity covers how to recognize changes in a patient's physical, cognitive, emotional, and functional baseline; how to communicate findings clearly and promptly to the nursing team using structured frameworks such as SBAR; and how to document observations accurately in paper and electronic health records. The activity also addresses normal versus abnormal findings within CNA scope of practice, nonverbal pain recognition, pressure injury identification, delirium awareness, and the CNA's role in team-based and patient-centered care.
