The main objective for health care systems and primary care settings is to provide quality patient care. Patient outcomes are directly linked to a healthcare team's ability to reduce fragmented care while increasing effective communication and continuity or care throughout all patient care transitions. In this presentation, Lesa Schlatman will provide education that defines the core components of continuity of care as well as the impact continuity can bring to both the patient and the healthcare settings. Lesa will also show attendees the importance of identifying healthcare partners and effective processes that will promote collaboration and communication across the care continuum that will ultimately lead to overall continuity of care.
