Accurate, timely, and objective documentation is an essential component of safe nursing practice. The health record communicates the patient's condition, nursing assessments, interventions, responses to care, provider communications, and follow-up while supporting continuity of care and professional accountability. This 1-hour continuing education course reviews essential principles of nursing documentation with an emphasis on patient safety, clinical communication, professional accountability, and risk reduction. Participants will examine characteristics of effective documentation, common documentation errors, appropriate handling of late or corrected entries, and documentation considerations involving abnormal findings, patient education, refusal of care, escalation of concerns, and adverse events. Case-based examples will allow learners to identify documentation deficiencies and apply evidence-informed documentation principles to situations encountered across nursing practice settings. Upon successful completion of the course requirements, eligible nursing professionals will receive 1.0 continuing education contact hour.
