Nurses have a vital role in improving the quality, accessibility and value of healthcare across the continuum of care. Nurses serving in primary care roles could expand access to care, educate people about health risks, promote healthy lifestyles and behaviors to prevent disease, help ameliorate the serious consequences of chronic diseases and coordinate care for the most frail in our society. Nurses can both contribute to and lead interprofessional teams that deliver person- and family-centered care. This module outlines progress made toward implementing the recommendations set out in the Institute of Medicine report The Future of Nursing, Leading Change, Advancing Health.
