Interdisciplinary Care Planning requires members of the Interdisciplinary Treatment Team (IDT) to view the Care Plan as a resident centered collaborative effort - rather than as a staff centered document. Learn how to utilize the Care Plan to truly function as a team - working together to enhance the care and quality of resident life. Individualize Care Plans to reflect the needs, interests, customary routines and personal preferences of individual residents. Develop Resident Centered Care Plan Problems. Write Care Plan Goals which are specific, measurable and resident centered. Write Care Plan Approaches which are staff centered yet reflect the residents individualized needs, interests, routines and preferences. Identify behavioral (non-drug) alternatives to pharmacological intervention and physical restraint. Identify the "HOT 10" Care Plan Problems which MUST be interdisciplinary - and how to ensure that they are!
