Course overview
An incident report (IR; also called accident report and occurrence report) is a written, confidential record of the details of an unexpected occurrence (e.g., a patient fall, administration of the wrong medication) or sentinel event (i.e., defined by The Joint Commission [TJC] as an unexpected occurrence involving death or serious physical or psychological injury, or the risk thereof). This continuing education module is designed to give the reader/learner an increased understanding of how to write an IR. One contact hour of continuing education credit is available for those who successfully complete the post-test and evaluation form.
