CASE MANAGEMENT: DISCHARGE PLANNING

Course overview

Discharge planning begins the first day of the inpatient stay when the patient initially enters the hospital system. This continuing education module is designed to give the reader/learner an increased understanding of discharge planning in case management, including the role of the case manager, challenges with discharge planning, potential risks associated with inadequate communication, and recommendations to ease the transition from hospital to home or other facility. Readers may explore any or all of the topics. One contact hour of continuing education credit is available for those who successfully complete the post-test and evaluation form.
