Course overview

Self-management is a concept that places emphasis on the patient¿s active participation in shared decision making, including helping to define what health conditions will be addressed and what treatment will be considered. This continuing education module is designed to give the reader/learner an increased understanding of the procedure for providing patient education about self-management of chronic disease, including a step-by-step process and the benefits of patient education on this topic.
